How it Works

For Birth Certificate

For an individual 18 years or older, provide a current government issued photo ID (i.e. Drivers license, state ID, passport, and armed services card). A parent needs to be present and provide ID to obtain a copy of a birth certificate for someone under the age of 18.

Fill out birth record request form and provide contact phone number.

Usually within 7 business days, you will receive a phone call to pick up your copy (copies) of the birth certificate.

For Marriage Certificate

Provide a current government issued photo ID (i.e. Drivers license, state ID, passport, and armed services card). Only one party needs to be present.

Fill out marriage certificate request form and provide contact phone number.

Usually within 7 business days, you will receive a phone call to pick up your copy (copies) of the marriage certificate.

For Death Certificate

Provide a current government issued photo ID (i.e. Drivers license, state ID, passport, and armed services card). You must be a member of the deceased’s family.

Fill out the death certificate request form and provide contact phone number.

Usually within 7 business days, you will receive a phone call to pick up your copy (copies) of the death certificate.
